We hope that the following information will make the process of starting physical therapy go more smoothly for you. As a new patient, there are many things we need/want to know about you. Simply follow the steps below and we will see you soon.
STEP ONE: Call us for Insurance Verification
Insurance benefits can be somewhat complicated and overwhelming to deal with. As a convenience for our patients, we will call your insurance company to verify your benefits and then explain them to you prior to starting physical therapy. Simply call the clinic with your insurance card available and our intake specialist will ask you a few questions. We will call your insurance company to verify your benefits and call you back to explain them to you and schedule your appointments.
STEP TWO: Print and Complete the Patient Intake Forms

The patient intake forms will provide us with pertinent information regarding your medical history and current symptoms as well as contact information for our records.
STEP THREE: Print and Complete the Functional Scale Pertaining to your Specific Injury

The functional scale will help us evaluate and treat you based on your specific injury and needs.
STEP FOUR: Come in for Therapy
If you have printed out and completed the forms above, you can arrive 10 minutes before your appointment time. If you would rather fill out the forms in our office, we suggest you arrive for your first session 15-20 minutes early to ensure we have all the time we need for assessment in your first session.
What to Expect:
During your first session, we will review your medical history and current issues with you and try to get an idea of your personal goals and expectations. Please bring loose fitting clothing. Men can wear a t-shirt and athletic shorts and we request that women bring a sports bra and athletic shorts. After reviewing your medical history, we will perform physical testing to gain a better idea of the physical factors underlying your symptoms. Upon completion of the physical testing, we will have theory of what is creating or perpetuating your symptoms and we will develop a plan for how to address those issues. By the end of your first session, we will have a plan of attack for correcting your problem and will have an initial picture of you as an individual. Every patient is an individual and we seek to find the most effective strategy to get you back to an active lifestyle quickly.
